Chuckanut Valley Veterinary Clinic High School Scholarship

Dear Senior,

Congratulations! Soon you well be graduating from high school and stepping out into another very exciting chapter of life.

We invite you to participate in the Fourth Annual $500 Chuckanut Valley Veterinary Clinic Scholarship, which is to be granted to a student who desires to pursue studies toward a career working with animals. This scholarship is focused on student service in the community, achievements, future plans and academic standings.

We invite you to submit an application.

Scholarship Requirements:

· Must be a senior graduating in 2009.

· Must be continuing education by going to a two or four year college, a vocational school, etc.

· Must write a five hundred-word essay, explaining what has influenced your decision to pursue studies toward a career working with animals.

· Compile additional information as stated on the attached CVVC Scholarship Application and Questionnaire sheet.

· Ability to provide, if granted scholarship, an acceptance letter to college or school you will be attending. Scholarship funds are to be applied to tuition. Inability to provide confirmation will result in scholarship being granted to next runner up.

The completed application must be mailed or delivered in a sealed envelope to the following address arriving no later than April 20, 2009, by 4:30 pm. There will be no exceptions.
