Elite Pet Membership Program for Puppy / Kitten Patients
Our Doctors and staff enjoy a reputation for thoroughness. In our modern veterinary facility, we provide individual care for each patient and personal attention to the questions and concerns that you might have. We will strive to keep you informed of the latest advances in veterinary medicine and how they pertain to your pet.
We are dedicated to keeping your pet healthy and comfortable so that you may enjoy the special wonders a pet can add to your life. We would like to recommend a special membership program for your pet that provides ultimate care for their first year at CVVC.
Our CVVC Elite Pet Membership for puppies and kittens include:
· All necessary vaccines pertaining to their first year.
· Initial exam and relative visits pertaining to recommended vaccine schedule (Maximum three vaccine visits).
· Fecal exam for internal parasite identification.
· One free additional office visit during the year.
· Ovariohysterectomy (spay) or Castration (neuter). If you are not planning on spaying/neutering we will include a preliminary OFA evaluation for hips and elbows.
· Complete pre-anesthetic blood work.
CVVC Elite Membership Cost:
Female puppy: $46/month
Male puppy: $38/month
Female kitten: $38/month
Male kitten: $29/month
At point of purchase you will make your first monthly premium at check out. All additional monthly payments are via bank draft or credit card for the next 11 months. Payments will occur on the 5th, or next business day, of each month until paid in full.

· Semi-annual visit: Your pet’s health status can change rapidly over a years time frame. Examining every six months will help ensure we identify all conditions early for prompt treatment.

The monthly cost is only $15 per month. At point of purchase you will make your first monthly premium at check out. All additional monthly payments are via bank draft or credit card for the next 11 months. Payments will occur on the 5th, or next business day, of each month until paid in full.

Our CVVC Elite Pet Membership for geriatric patients include:
· Annual examination.
· All necessary vaccines pertaining to your pets lifestyle
(Rabies, Kennel Cough, DHP, and Lepto).
· Fecal exam for internal parasite identification.
· Comprehensive blood work will be sent to Idexx Reference Lab: CBC, Chemistry Profile, and Thyroid Test. These tests will help ensure that your pet’s internal organs are functioning normally. Early detection of organ failure is key to successful treatment.
· Urinalysis: Mary diseases can be diagnosed via testing of the urine. It is an important part of a complete health screen.
· EKG: Is the measurement of electrical activity of the heart. We send your pet’s EGK to a cardiologist via the phone line. The cardiologist will evaluate the EKG tracing and give us a report.
· Semi-annual exam: Your pet’s health status can change rapidly over a year’s time frame. Examining every six months will help ensure we identify all conditions early for prompt treatment.
